To improve the confidence building cooperation in the health sector in the Transnistria region.

The current social and healthcare systems in the breakaway Transnistria region need to respond to new approaches and emerging demands in prevention and care oriented to lay people and people with chronic disabilities / diseases, as well as aged people with Long-Term Care (LTC) needs, who do not necessarily have to be hospitalized to receive specialized and non-specialized care services.

The increasing ageing and thus increasing demand in hospitalisation and social care of this category of population, considerably challenges the healthcare and social assistance systems with their limited budgetary allocations. Thus, the Home-based LTC - medical and social care at home - would represent a solution to cover the increasing needs for LTC at relatively low costs.

In line with above, the Project intends to assist Transnistrian relevant sectors in applying informal mechanisms in the primary care, based on specific needs of bed-ridden / home-dependent people, to be able to meet their basic needs, and to help the domestic and / or unprofessional caregivers who face difficulties in the process of medico-social (integrated) care provision at home.


The Project underlays a targeted cooperation in the health and social sectors from both sides of Dniester river aimed at implementing a long-term Change Process in the existing health and social care systems focused on disabled and lay people specifically in Transnistria region.

Transnistria region 

Direct Beneficiaries:

  • People with long-term care needs.

  • Health centres’ managers, doctors, nurses, public health specialists, social assistants and social workers in Moldova, especially in the TN.

  • Family members, potential volunteers, NGOs dealing with people with long-term care needs.

  • Health institutions in Moldova, especially of the TN’s primary care.


Indirect Beneficiaries:

  • Policy makers in reintegration and health / social areas, namely: Bureau of Reintegration Policies (BoR) under the State Chancellery of the Republic of Moldova and the Ministry of Health, Labour and Social Protection (MHLSP).

  • Health and social actors in the Transnistrian region.

Expected Results:

  • Improved qualifications in homecare - Curricula and Handbook for caregivers on both riverbanks

  • Developed capacities in homecare – training of 70 local trainers and up to 240 domestic caregivers. Up to 500 patients served by local trainers.

  • Cross-river cooperation – creating improved conditions for exchange between riversides based on established Day Care Centre
